Explanation of forms

TN Certificate of Immunization: www.bryanpolyclinic.com
The TN Certificate of Immunization form is needed to meet American College Health Association requirements which state that we must have students immunization records on file. Please note that all new students must prove immunity to measles and rubella; and all new students must have had a tetanus shot within the last 10 years. A complete physical is required ONLY for those students planning to participate in an intercollegiate sport and that will be given at Bryan College after the athletes arrive.
Meningococcal Meningitis and Hepatitis B: www.bryanpolyclinic.com
The Higher Education Commission of the State of Tennessee has required that all public and private institutions of higher education provide notice to newly enrolling students concerning meningococcal disease and hepatitis B. These vaccinations are not required for admission; however, you must complete and present this form to complete registration.

The Office of Student Life will use the information on this form to assign you to a compatible roommate. If you have made arrangements to room with someone specific, please indicate that on the form. Such requests will be honored whenever possible. Housing assignments will begin mid-July for students that have made their $200.00 deposit. No room assignments will be made until the deposit has been received. ($100 enrollment deposit + $100 room deposit) Commuters need only pay the enrollment deposit of $100.

All students are required to have adequate health insurance that is valid in the state of Tennessee and will be billed for coverage under the Bryan College Group Student Health Insurance plan unless proof of other valid insurance is provided. There will be NO exceptions. To waive out of the College’s plan you must complete the Student Information Form and provide a photocopy (front and back) of your insurance card or you will be automatically covered under and billed $715.00 for the College’s plan. If purchasing this insurance in the spring semester the insurance cost is $447.
**Please check with your Insurance provider making sure it is an insurance that accepts our student health physician and physicians in the Dayton, TN area as a participating provider.

VEHICLE REGISTRATION: https://www.bryan.edu/parking_registration.html .
(Required for anyone planning to have a car on campus. All vehicles, belonging to on or off campus students, that will be parked on campus for any reason, must be registered and have a parking permit) Please indicate if you want the Standard Parking (Resident Student) or Commuter Parking (Off Campus student)(Gold, Burgundy, Green, or Commuter) at $50.00 per permit for the year. The charges will be added to your school bill and can be paid in the Business Office. The actual permit will be distributed with the fall matriculation material.
