Required Forms + New Student Information + Student Life

The TN Certificate of Immunization form is needed to meet American College Health Association requirements which state that we must have students immunization records on file. Please note that all new students must prove immunity to measles and rubella; and all new students must have had a tetanus shot within the last 10 years.
